The utilities changebar and nrchbar allow insertion of changebars in groff source.

A hacked version of nrchbar, that works with cvs diff output, can be found at http://home.windstream.net/kollar/groff/nrchbar.tar.gz - you might use it like this:

 cvs diff foo.ms | nrchbar -c foo.ms

 rcsdiff foo.ms &> diff; nrchbar -c foo.ms < diff
